Who Qualifies

To be eligible for the Agency with Choice / Alternative Day Program, individuals must:

Be DDS eligible
Have Chapter 688 or DDS funding allocation
Be referred by a DDS Area Office Transition Coordinator or Adult Service Coordinator

This program is ideal for individuals who:

Want more flexibility and control over their services and schedule.
Prefer to hire and manage their own staff
Do not require full daytime coverage (30 hours)
Are willing to take on more responsibility in exchange for independence

Participants can build their weekly plan with Case Management support

Volunteering – community service or job exploration
