You’re Almost There — Just a Few Final Steps Before Joining MCS

Congratulations on completing your interview! We’re excited to welcome you closer to becoming part of the MCS family. Before we can move forward with orientation and onboarding, we need a few required forms completed to verify eligibility and ensure compliance with state regulations.

These forms help us maintain a safe, supportive, and trustworthy environment for the individuals and families we serve. Please complete each item below as soon as possible.

Your Orientation Steps

CORI Request

A mandatory background check for all prospective employees.

DPPC Abuser Registry Consent Form

Required to verify you are not listed in the DPPC Abuse Registry

Driving Record Authorization

Needed for any role that may involve transporting individuals or driving on behalf of MCS.

Additional Requirement for DPH-Certified Programs

If the position you are being considered for is within any DPH-Certified program, you must also complete the following

MAP Registration Verification Authorization Form

This verifies eligibility for Medication Administration Program (MAP) training, if required for your role.

What Happens Next?

After your forms are submitted:

  • HR will review your documents.
  • You will receive an email with the next steps and your Orientation instructions.
  • Once cleared, you’ll officially move into the onboarding stage and be closer to becoming an MCS team member!
