Required Onboarding Packets

New employees must complete all three orientation packets. When you click a packet button, you’ll be redirected either to the DoForms website to acknowledge and sign MCS forms. All packets must be completed before you begin work.

Introduction & Starting Packet

This packet includes essential onboarding forms, such as your direct deposit setup, emergency contact information, handbook acknowledgment, payroll status change documentation, and the MCS mission and strategic plan. Please complete this packet first.

MCS Policies Packet

This packet contains all required policy acknowledgments, including confidentiality, ethical conduct, GPS and transportation policies, leave laws, reimbursement guidelines, and training requirements. Review each policy carefully and complete all required signatures.

Compliance Packet

This packet includes mandatory compliance acknowledgments such as corporate compliance, responsibility standards, DDS requirements, DPPC/mandated reporting, human rights acknowledgment, and workers’ compensation information.

Complete Your Bloodborne Pathogens Training

All MCS employees must complete this training before beginning direct support work. Start by reviewing the training document, then take the test to verify your understanding.
