Case Manager - Placement Services

Hours:

Full Time – 40 hrs/wk

Summary:

Responsiblitlies:

Ensure compliance (in cooperation with the Care Provider) in the following areas:

  • ISP requirements - Prepare assessments for the ISP, including goals, strategies, and updates.
  • All Health and safety needs of the consumer.
  • All Quest requirements and documentation.
  • Routine visits at caregivers’ homes to provide ongoing support and monitoring.
  • Routine visits with the consumer to assess ongoing quality and satisfaction with supports.
  • Coordination of clinical and medical supports in cooperation with the MCS RN, individual, and Care Provider;
  • Recruitment of direct support staff in cooperation with the individual and Care Provider.
  • Supervision of direct support staff.
  • Ensure the capacity of the program to provide support to Individuals from different cultures.
  • Represent MCS as requested within the community and other agencies /organizations with similar objectives.
  • Attends staff meetings regularly and specific training related to individual consumers' needs.

Required Qualifications/Skills/Training:

The Case Manager must have a bachelor’s degree or a minimum of 5 years of recent professional healthcare experience in a related field. Case Manager must also have a valid driver’s license and access to an automobile.
