Responsiblitlies:
1. Conduct initial and ongoing assessments of the AFC qualified setting. This includes doing a home inspection to evaluate whether the home is appropriate for the member.
2. Obtain a social history and conduct a psychosocial assessment and evaluation.
3. Participate in the development, implementation, and ongoing review of the AFC plan of care.
4. Select, train, evaluate, and supervise AFC caregivers in conjunction with the registered nurse.
5. Conduct on-site visits (home visits) with each member at the qualified setting bi-monthly (alternating with the bimonthly visit by the registered nurse) for Level I, and monthly for Level II.
6. Complete a care-management progress note corresponding with each on-site visit or encounter and upon significant change.
7. Periodically review AFC caregiver logs.
8. Assist with obtaining information and accessing other healthcare and community services.
9. Review and document the fire and safety procedures for the qualified setting. This includes acquiring a fire escape plan (both written and drawn) from the designated caregiver.
10. Participate in discharge planning and implementation
11. Review the suitability of the qualified setting at least annually.
12. Represent MCS as requested within the community and with other agencies/organizations with similar objectives.
13. Attends staff meetings regularly and specific training related to individual consumer’s needs.
