Family Bridges Service Navigator

Location: MCS, 505 E. Columbus Ave., Springfield

Supervisor: Isabel Castro

Hours: One position 40 hours/week – Full-time. One position 20 – 30 hours/week – Part-time

Responsibilities: Both Service Navigators will be cross-trained to work in the 3 programs listed above.

The service navigator will work with a variety of families including but not limited to families who have a member living in their home who has complex medical challenges and needs; families who need services to prevent out of home placement and families who are seeking support to navigate the DDS eligibility application process. Supports include but are not limited to, case management, accessing generic services, assessing needs, developing Family Support Plans, and providing information and referral services.

Additional responsibilities include:

    • Maintains quality of programming through adherence to agency standards and DDS regulations.
    • Service Navigation for the individual/family
    • Where applicable, hiring and supervising direct care staff
    • Work closely with DDS Service Coordinators
    • Outreach to generic services and collaboration with other state agencies and service providers
    • Maintain accurate financial records
    • Ensure that all documentation is completed on a timely basis, and other duties as required
    • Work with school districts to provide continuity of services from school to home.

Qualifications: BS or BA in a related field with one-year experience preferred. A minimum of High School Diploma with 3 years of experience in the field of Human Services. Preference is given to a qualified individual who is a family member of a child/adult with complex medical challenges, as their own life experiences provide them with invaluable information and unique ways to build connections with the families to whom they are providing supports. Bilingual (English/Spanish) a plus. Must have a valid driver’s license and dependable vehicle.
